Homeschooling County/City Notification Form

If your child is new to homeschooling, you will need to submit a Homeschooling Notification Form to your local school jurisdiction homeschool contact person.

Make sure your form is received at least  15 days before the beginning of the school year, (It’s best to send it by August 10th, to be on the safe side, if using snail mail).
Currently, this form only needs to be filled out when a child is first enrolled in a homeschool program, as long as the child stays continuously enrolled in a similar legal state-recognized home instruction program. There is no cost when you submit this form.
The notification form is where you will list “Lubavitch Home-School Umbrella Program” as your umbrella group, and also where you will tell the school system if you want your child to participate in State level standardized testing.

This is not required, but if you opt in, this testing must be arranged by contacting the principal of your specific public school system.  This does not include SATs or other federal level testing.   If you say on this form (homeschool notification form) that you want to participate in testing, (by selecting Part B, 2A on the form), please note that you will need to contact schools (principal) in your jurisdiction during February of each year regarding the testing for grades K-5, and in September to arrange testing for 8th graders.  There are no state standardized tests for grades 6,7, 9, 10, 11 or 12.       If your child participates in testing, tests will be submitted to the Maryland State Department of Education for grading and in turn, test results will be mailed directly to parents.

LHUG Review Form – This form should be filled out separately for each child at least once a year for the purpose of documenting that LHUG has reviewed each child’s program of instruction, as per our certification to the state.  Any information shared is part of a private record and does not go to the state.   This simple review process can serve as a check-in for a parent to notice what to stop, start, shift or continue in their home instruction program, particularly in the required subject areas.   To keep this simple, the form may be filled out at any place where the child is receiving instruction, which includes any classes, trips or any home visits that occur for any reason.   This form is for your records only, it can serve as notes to talk about at your end-of-the-year review meeting
